Community Connect Program
Community Connect is committed to supporting individuals and families─ including children with special healthcare needs─ by walking alongside them as they navigate essential services. We build trust through personal, culturally responsive connections and help people access local resources that reflect their language, values, and lived experiences. Our care coordinators provide one-on-one guidance, helping individuals and families understand their options and overcome barriers such as language, transportation, or unfamiliar systems. By connecting people to the support they need, Community Connect strengthens families, promotes dignity and well-being, and addresses the social and economic factors that influence health.
Care Coordinators can offer the following services:
- Case management
- Connection to community resources
- Assistance with applying for programs
- Assess needs and goals
- Guidance to achieve stability and improved health
